Here are the authentication forms for external assessment 2006. Click here for the NZQA link Information regarding the preparation for the external assessment can be found in the following documents: Schools are required to verify that the material presented for external assessment is the candidate's own. For Level 1 and 2, teachers and students are required to complete and sign the Authenticy Form on the back of the portfolio folders.
